Packing Declaration Template May 2021
This is for the packing material used in shipping your goods, including any pallets it may be based on. This form must be completed in english, be on your suppliers letterhead, be signed and dated, and be free from eraser and alterations.
This is a single shipment packing declaration and valid only for the shipment listed on the declaration.

Annual Packing Declaration May 2021
This is for the packing material used in shipping your goods, including any pallets it may be based on. This form must be completed in english, be on your suppliers letterhead, be signed and dated, and be free from eraser and alterations. When approved, it will be valid for 12 months from the date of the form.
Plywood Declaration
This is for any plywood material, including any pallets or crates it may be based on. This form must be completed in english, be on your suppliers letterhead, be signed and dated, and be free from eraser and alterations. Plywood materials shipped which aare manufactured within 90 days of shipping may be released with an acceptable plydoow declaration.
EFT Registration Form
This registers your account details with Australian Customs Service.
Shippers Letter of Instruction (SLI)
To be completed for export shipments for air & sea from Australia. When completed please attach the applicable commercial invoice.
EFT Direct Debit Request Form
Form requesting Australian Customs to EFT direct debit charges.
Trading Conditions

Fumigation Treatment Certificate
This form is required whenever there are any wooden components in your shipment, including any packaging materials, being crating or palleting.
Unaccompanied Personal Effects Statement
This form needs to be completed for any shipments where the goods are your own personal belongings, that you have owned for over 12 months, that are returning back form overseas, whether you have been travelling, or living abroad. This form must be presented with a copy of your passport.
Heat Treatment Certificate
This is another form of treatment for wooden components as described in Fumigation Certificates.
